If the back of your heel feels stiff on your first steps in the morning, loosens up briefly once you start moving, and then hurts again in the afternoon, Achilles tendinitis—a state of accumulated repetitive load on the Achilles tendon—should be suspected before plantar fasciitis. Plantar fasciitis can also hurt in the morning, but the pain starts in a different location.

With Achilles tendinitis, the back of the heel feels stiff and painful the first time you move after rest. Taking the first few steps after getting out of bed, the back of the heel may feel like it's pulling or stabbing, but as the tendon adjusts to movement, it can loosen up briefly. Because of this, the pain often feels tolerable by the time you head to work.

As walking and standing time increase and you repeatedly climb stairs, calf force keeps transferring to the Achilles tendon. What Happens When Load Accumulates on the Achilles Tendon

The Achilles tendon transmits the force generated by the calf muscles to the heel bone. This tendon is pulled strongly when you push off the ground while walking or stand on your toes. If you suddenly increase running, jumping, or uphill walking, the tendon inevitably gets stimulated more often than it has time to recover.

In the early stages, rest may ease the discomfort. Once the balance between load and recovery breaks down, problems can arise in how the tendon adapts, and the tissue's arrangement and thickness may change. Pain at the point where the Achilles tendon attaches to the heel bone is classified as insertional tendinopathy. Conversely, thickening or tenderness a few centimeters above the heel bone points more toward midportion tendinopathy.

Strength changes as well, not just pain. If you used to do many single-leg heel raises but now your heel drops after just a few repetitions, or you need to shorten your stride to walk, tendon function may have declined. Insertional and midportion tendinopathy differ not only in the location of pain but also in where the tendon is compressed and how much it is stretched. Where Do Achilles Tendinitis and Sole Pain Diverge?

The sole of the foot and the back of the heel have different structures where pain originates. With plantar fasciitis, pain is usually most noticeable when pressing on the bottom of the heel, particularly the inner side. Because the plantar fascia extends from the heel bone toward the toes, a pulling sensation can also run along the sole.

Achilles tendinitis causes pain behind the heel. Pressing a finger on the spot where the tendon attaches to the heel bone, or just above it, produces tenderness, and the pain worsens when the area rubs against the back of a shoe or when the calf muscle contracts. Simply identifying whether the discomfort sits under the sole or behind the heel gives a clue for telling the two conditions apart.

Timing alone is not enough to distinguish them. Both conditions can cause stiffness with the first steps in the morning or after standing up from sitting for a long time. With Achilles tendinopathy, the stiffness may ease after a few steps, but the pain behind the heel often returns in the afternoon once walking and climbing stairs have accumulated. Some people have pain in both the sole and the back of the heel at the same time. Grouping these symptoms under a single condition can cause the cause of the remaining pain to be missed. Clinicians examine the inner sole of the heel, the Achilles tendon attachment site, and the tendon above it separately, and record where pain occurs when the toes are bent back and when standing on the toes.

If a snapping sensation or sound occurs behind the heel at the moment of suddenly changing direction or jumping, this needs to be distinguished from the usual overuse pain. If standing on the toes afterward becomes difficult and the ability to push off the ground drops sharply, testing for an Achilles tendon rupture is needed. What Does the Clinical Exam Check?

Clinicians ask when the pain started and check whether running distance, walking time, or stair use increased around that time. They also ask whether the patient's usual shoes have a stiff heel counter and whether symptoms began after switching to new shoes. They record specific details such as how many minutes of morning stiffness the patient experiences and which afternoon activity makes the pain worse again.

The physical examination covers several distinct sites. Clinicians press on the point where the tendon attaches to the heel bone (calcaneus), then compare tenderness and thickness along the tendon further up. As the patient moves the ankle up and down, they record where and how severely pain occurs with each motion, and they also have the patient repeat single-leg heel raises to measure how much the height and number of repetitions differ from the opposite side.

Clinical examination forms the core of diagnosis. Clinicians often diagnose Achilles tendinopathy by asking about the patient's symptoms and examining the tendon directly.(von Rickenbach Kristian J, 2021) If there has been a sudden loss of function, they first rule out a possible rupture by checking whether the ankle moves when the calf is squeezed.

Musculoskeletal ultrasound provides imaging support for the lesion found on examination. Clinicians check the ultrasound image for tendon thickening, disrupted fiber alignment, partial tears, or changes in surrounding tissue. It can also allow real-time comparison of how the tendon and nearby structures contact each other during movement.

The insertional type causes chronic heel pain where the heel bone meets the Achilles tendon.(Chen Sheng-Hao, 2022) When a clearer picture of the shape of the protruding heel bone or the structure where bone and tendon meet is needed, an X-ray is taken. Once the location of the lesion is identified, the patient reduces walking distance and stair use from that day. If a rupture is suspected, walking is stopped first and detailed testing is carried out before anything else.

How Do You Adjust Load and What Treatments Get Added?

Treatment starts with adjusting activity to a level the Achilles tendon can tolerate. Running, jumping, uphill walking, and stair climbing—activities that repeatedly aggravate the pain—get scaled back for a while. If a stiff shoe heel presses on the insertion site, switching to shoes that put less pressure there helps, and rather than resting all day, the day's activity level is set by tracking walking and heel-raise function.

In the insertional type, compression between the tendon and the heel bone can increase when the ankle bends deeply. In the mid-portion type, repeated tensile load and tendon function are measured more closely. The care team compares morning stiffness duration, afternoon pain, the number of single-leg heel raises, and walking distance against previous records, then raises or lowers activity level accordingly. If pain and functional decline continue even after adjusting load, musculoskeletal ultrasound is used again to recheck the lesion location and tissue condition. For insertional Achilles tendinopathy, shockwave therapy may be considered as a non-surgical option alongside load management, and the care team decides whether to proceed based on the lesion location and symptoms.(Chen Sheng-Hao, 2022)

When the tendon itself also needs to be addressed for recovery, prolotherapy or ozone injections are given under ultrasound guidance. This approach stimulates the ligament and tendon to regenerate, and the needle tip position is checked on screen as it is inserted. When tight calf muscles are placing heavy load on the tendon, Winback radiofrequency and Lympha taping are used together to release tension in the calf and plantar fascia.

Going back to the previous exercise volume right away just because pain has decreased is more than the tendon can handle.
