When examination confirms cervical musculoskeletal pain and restricted range of motion, therapeutic exercise aimed at pain relief and functional recovery is the starting point. If the range of motion is too limited for exercise to be tolerable, manual therapy (hands-on treatment to mobilize stiff joints and release tight fascia) is used first to restore that range. For example, comparing whether meeting content that used to be easy to follow for 50 minutes is now lost after 20 minutes is far more informative than a general impression. Whether someone who used to wake once a night is now waking four times, or whether drowsiness that used to arrive at 3 p.m. is now starting in the early morning, should all be part of the clinical record.

The clinician uses the pace of symptom change to decide whether to reinterpret existing test results or add new tests, and then determines which area — sleep, medications, metabolic health, or neurological function — should be prioritized next. A structural brain scan that found no abnormality is genuinely useful information, but it does not make the fogginess and difficulty concentrating disappear.

Tracking when symptoms worsen and which specific functions have declined allows the next assessment and treatment steps to be shaped without reflexively repeating the same tests.
