You've Seen Every Specialist and They All Say You're Fine
Fatigue, dizziness, and palpitations that overlap — yet every test comes back normal — can leave you feeling unheard and without answers. Even when organ-specific tests for the heart or digestive system are normal, the autonomic nervous system that regulates heart rate, digestion, sleep, and body temperature may still be functioning poorly. Dysautonomia is less a single clear-cut diagnosis and more a clinical term that covers a range of autonomic regulatory problems.
Some people go through an ECG and echocardiogram for palpitations, an upper endoscopy for bloating, and consultations for sleep problems and anxiety — only to be told each result is normal. That means no structural disease was found in the heart or stomach. It does not mean the symptoms aren't real.
When one person's day is divided up by medical specialty, it's easy to miss the connections between symptoms. Morning fatigue, dizziness on the commute, and palpitations with abdominal bloating after dinner can each be treated in isolation. Rather than simply collecting individual test results, it helps to piece together a timeline — which symptom came first, and what followed.
When autonomic function is disrupted, problems can appear not just in the cardiovascular system but also in the gastrointestinal tract, sweat glands, temperature-regulation system, and bladder. Reading the pattern of a patient's history and symptoms — while ruling out other conditions — is an essential part of evaluation. It is a clinical term used to describe remaining symptoms through the lens of regulatory dysfunction.
What Does the Autonomic Nervous System Actually Control?
Without any conscious input, the autonomic nervous system regulates heart rate and blood vessel diameter, drives the movement of food through the stomach and intestines, and influences sweating, body temperature, saliva, tear production, and bladder function. Its activity also shifts with the sleep-wake cycle. When this regulation breaks down, multiple organs can become uncomfortable at once. (Baker Jacquie R et al., 2024)
When you climb stairs, the body increases heart rate and adjusts blood pressure to deliver more oxygen to working muscles. After a meal, blood flows toward the gut and digestive activity picks up. As night falls, heart rate and alertness both drop to prepare for sleep — and the body orchestrates all of this automatically, moment to moment.
The sympathetic nervous system raises heart rate and alertness during movement or stress; the parasympathetic nervous system creates the right conditions for digestion and recovery during rest and meals. Neither branch is inherently better — a sympathetic response is necessary when climbing stairs, but it also needs to settle down once you reach the top.
When that transition doesn't go smoothly, discomfort follows. You might lie down for bed with your heart still racing and your mind wide awake, or stand up and find your vision blurring because your blood vessels and heart rate didn't respond in time. Some people experience bloating, cold sweats, and sudden fatigue all at once after eating. These patterns alone, however, are not enough to confirm autonomic dysfunction.
Why Don't the Symptoms Point to One Place?
When the autonomic nervous system isn't working properly, symptoms can emerge across multiple organs. You might wake up exhausted, feel dizzy standing on the subway, and then have your heart suddenly pound hard for no clear reason. After lunch, your stomach feels uncomfortably full, your hands and feet go cold, and you wake up sweating several times during the night. (Hovaguimian Alexandra, 2023)
Seeking care for each symptom separately means undergoing an echocardiogram here and an endoscopy there — and being told that each organ's structure looks normal. An echocardiogram examines the heart's structure and movement; an endoscopy looks at the lining of the esophagus and stomach. These tests, however, don't capture the full process by which the body adjusts heart rate, blood vessels, digestion, and temperature to match each situation — so a normal result doesn't make the discomfort disappear.
Functional disorders are conditions where symptoms persist because of problems in how the body regulates itself, even without any visible structural damage. These should not be dismissed as purely psychological. Anxiety and poor sleep can destabilize autonomic responses, but there is no basis for concluding that a patient's dizziness or palpitations are symptoms created by the mind alone.
Different autonomic function tests also assess different things. A normal result on one test does not mean all autonomic functions are intact, and a single out-of-range value does not confirm dysautonomia. Clinicians review the posture and timing of symptoms, their relationship to meals and sleep, current medications, and physical examination findings together. (Cheshire William P et al., 2021)
Some Conditions Need to Be Ruled Out First
Before interpreting symptoms as autonomic dysfunction, clinicians first check for thyroid disorders, anemia, arrhythmias, sleep apnea, and medication effects. In patients with long-standing diabetes, diabetic autonomic neuropathy is also considered. When a cause is identified, treatment is adjusted to address that condition directly.
Hyperthyroidism (an overactive thyroid gland) produces palpitations, sweating, weight loss, and fatigue that closely resemble autonomic symptoms. (Lee Sun Y et al., 2023) An underactive thyroid can cause profound fatigue, cold sensitivity, and weight changes. A thyroid hormone blood test distinguishes between the two. Anemia levels and iron stores are also checked to see whether they might explain dizziness on standing or shortness of breath.
In diabetic cardiac autonomic neuropathy (nerve damage to the heart's regulatory system caused by diabetes), heart rate may not rise appropriately with exercise or position changes. (Agashe Shruti et al., 2018) Clinicians review how long a patient has had diabetes and how well blood sugar has been controlled. If cardiovascular responses appear abnormal, diabetes management and cardiac risk assessment are factored into the care plan.

What to Bring to Your Appointment
When fatigue, dizziness, and palpitations appear unpredictably, keeping brief notes at the moment they happen gives clinicians the information they need to spot patterns. The time of onset, your posture or activity at the time, whether it happened before or after a meal, and how long it lasted are enough. Jotting down any accompanying symptoms — nausea, cold sweats, blurred vision — alongside the previous night's sleep duration and that day's medications reduces the back-and-forth in the exam room.
There's no need to write at length. A single line works: "8:00 a.m., stood on subway for 10 minutes, dizziness and palpitations lasted 5 minutes, slept 4 hours the night before." Organized chronologically, these notes reveal whether symptoms repeat with position changes, cluster after meals, or worsen after a poor night's sleep.
Heart rate variability (HRV — a measure of how much the time between heartbeats varies) indirectly reflects how well the autonomic nervous system is regulating heart rate. (Baker Jacquie R et al., 2024) A high or low HRV reading alone does not confirm dysautonomia or gauge its severity. Breathing, sleep, caffeine intake, and medications at the time of measurement all affect the result. Clinicians interpret HRV alongside symptom records, medical history, and the exclusion of other conditions. (Cheshire William P et al., 2021)

A string of normal test results does not mean there is nothing left to investigate. HRV analyzes variation in the intervals between heartbeats to indirectly assess autonomic regulation — it is a supplementary measure, not a standalone diagnostic test. Because results are affected by measurement duration, posture, sleep, caffeine intake, and medications, HRV must be interpreted alongside symptom patterns and other test findings.
Q. If my heart tests are normal, is it safe to just monitor recurring palpitations?
Intermittent arrhythmias, anemia, thyroid dysfunction, or medication effects that weren't present at the time of testing may still be factors. If palpitations are accompanied by fainting, chest pain, or severe shortness of breath, or if they persist for a long time, further evaluation is warranted.
Q. Can smartwatch data be useful at a medical appointment?
Records that capture the time a symptom occurred, heart rate at that moment, and activity level can help identify recurring patterns. Smartwatch readings can contain measurement errors, however, and do not replace an ECG or Holter monitor (a device that records the heart's electrical activity over an extended period) performed in a medical setting.
Q. Do I need to stop my medications before autonomic testing?
Some cardiovascular medications, sleep aids, and anti-anxiety medications can affect heart rate and HRV. Do not stop any medication on your own — instead, write down the name of each medication and the time you take it so the results can be interpreted with that context in mind.
Q. If my symptoms worsen with position changes, what tests might be considered?
Repeated dizziness and palpitations when moving from lying down to standing can be evaluated by measuring blood pressure and heart rate in different positions. Depending on the situation, orthostatic vital sign measurements or a tilt table test (a test that monitors blood pressure and heart rate while the body is tilted from horizontal to upright) may be considered, and results are interpreted together with symptoms.
